No formal application, however an amusement ride may not be operated unless the operator or the operator’s designee submits notification to the department at least 30 days before the ride operates in the state for the first time. The notification must:
(1) be in writing;
(2) identify the ride or structure;
(3) specify the intended dates of operation and location of use; and
(4) be mailed, faxed or hand delivered to the department’s mechanical inspection section.
Inspection Criteria
For amusement devices, the department will inspect the ride or will designate a National Association of Amusement Ride Safety Officials (NAARSO) Level II Inspector to inspect the ride. Notification at least 30 days before operation is required by the state.

Inspection Criteria
State inspections are performed twice annually for permanent rides, while traveling rides are inspected at each set up. Click the link below for information about requesting a state inspection:

In addition to the state inspection, a third party inspector is required to provide an Affidavit of Compliance and Nondestructive Testing Form. This document needs to be completed no more than 60 days prior to permit date. Professional engineers registered in the state of Florida are considered qualified third party inspectors. Non-engineer inspectors must submit credentials to the State of Florida for approval prior to inspecting.

Inspection Criteria
Local authority having jurisdiction will complete inspection. Owner does have ability to hire third-party inspector. Third-party inspector must have Virginia State certification, which they can achieve by completing the Virginia State Certification Program found at the link below:

Inspection Criteria
All Inspectors must be certified by the West Virginia Division of Labor, and meet the training and experience requirements set forth in the ACCT Challenge Course and Canopy/Zip Line Tours Standards, Seventh Edition, Chapter 5, Section C, or equivalent.

Yes. A submission procedure exists, but specific forms are not required.
“Prior to initial public operation or after any modification of an Aerial Adventure Course authorized by a special use authorization, require the holder to submit required plans, designs, calculations, specification, related data, and certifications, including certification by a qualified engineer that the aerial adventure course meets ASTM 2959 or equivalent industry standard.” Note: The equivalent industry Standard must be determined by the Engineer of Record.

Inspection Criteria
The USFS permit holder is responsible for ensuring the following inspections/audits are completed on the Aerial Adventure Course(s) prior to opening and during each year of operation:
- Course(s) structure must be inspected/audited by an independent qualified inspector, for compliance with the ASTM 2959 Standards, at the permit holder’s expense prior to annual public operation.
- Course operations must be audited annually. A report of that audit and a written certification statement, by the qualified inspector, must be available for USFS review.
Designated USFS ropeway team member(s) will be onsite to monitor both inspections for initial commissioning of the course(s). Once in operation, a designated USFS ropeway team member will continue to monitor the course annually to ensure compliance.
